To maintain excellent quality of service and a well trained staff, we depend on your assistance and cooperation in financial matters. To assist you in properly communicating with your insurance company, please furnish our office with a completed registration form, insurance form, and a copy of your insurance card at the time of your initial appointment. Patient fee responsibility will also be explained at this time. You should be prepared to pay for insurance co-payments, pregnancy tests, injections, and any other services not covered at the time of your visit.

Our office has contractual provider agreements with most of the larger health plans in the State of Ohio. Both the customer service department of your insurance company and our office should be able to inform you about whether or not our providers are accepted by your health care plan. We will submit to both primary and secondary insurances for obstetrical services and surgical procedures. It is your obligation to pay the balance of the bill.

Kathy or Dolly, our billing experts, will be happy to discuss fees and/or insurance plans, or to answer any billing questions. If financial problems exist, every effort will be made to assist you with a payment plan.
